Renal dosing, restrictions and evidence limits

For atopic dermatitis with eGFR 15 to less than 30 mL/min/1.73 m2, the recommended dose is 15 mg once daily. For ulcerative colitis or Crohn disease in that renal range, use 30 mg once daily for induction and 15 mg once daily for maintenance; indication-specific induction duration requires the complete label. Use for these indications is not recommended below eGFR 15. Adult rheumatoid arthritis, psoriatic arthritis and axial spondyloarthritis do not require renal dose adjustment for mild through severe impairment in this source.
