Renal regimen and formulation
For CrCl above 50 through 80, the labeled regimen remains immediate-release 5 mg twice daily or XR 11 mg once daily. At CrCl 30-50 or below 30, use immediate-release 5 mg once daily; patients taking XR should receive the IR product rather than a guessed XR reduction.
Hemodialysis timing
On hemodialysis days, give the dose after dialysis. If it was taken before dialysis, do not give a supplemental dose afterward. These rules do not replace the distinct ulcerative-colitis regimen.
