Indication and usual adult dose
AGGRASTAT for non-ST-elevation acute coronary syndrome reduces the combined thrombotic cardiovascular endpoint of death, MI, refractory ischemia or repeat cardiac procedure. IV only: 25 micrograms/kg within 5 minutes, then 0.15 micrograms/kg/min for up to 18 hours.
Original preparation and compatibility instructions
Inspect for particles/discoloration; use only clear solution with intact seal. Remove the INTRAVIA foil overpouch; temporary plastic opacity from sterilization moisture may diminish. Squeeze the inner bag to check for leaks; discard if leaking or sterility is questionable. Do not connect bags in series because of air-embolism risk.
Withdraw the bolus from the 15 mL premixed bolus vial, or use the 100 mL premixed vial/bags. Do not dilute. Administer by syringe or IV pump within 5 minutes, then immediately begin maintenance from the 100 mL vial/bag using a pump. Discard remaining solution.
Bolus volume: 25 micrograms/kg x body weight / 250 micrograms/mL for the 15 mL vial, or divide by 50 micrograms/mL for the 100 mL vial/bag.
For CrCl above 60, maintenance mL/hour = 0.15 micrograms/kg/min x weight x 60 / 50 micrograms/mL.
The original same-line compatibility list includes heparin, atropine sulfate, dobutamine, dopamine, epinephrine HCl, famotidine injection, furosemide, lidocaine, midazolam HCl, morphine sulfate, nitroglycerin, potassium chloride and propranolol HCl. Do not share the line with diazepam. Do not add drugs to, or withdraw solution directly with a syringe from, the INTRAVIA bag.
Renal Dosing
CrCl at or below 60 mL/min: unchanged 25 micrograms/kg bolus within 5 minutes, then 0.075 micrograms/kg/min for up to 18 hours.
At 50 micrograms/mL, renal-adjusted mL/hour = 0.075 micrograms/kg/min x weight x 60 / 50 micrograms/mL.
