Usual dosing - separate products
Ticarcillin alone: 3 or 4 g every 4 or 6 hours (200-300 mg/kg/day).
Ticarcillin/clavulanate (Timentin): 3.1 g every 4-6 hours.
Renal Dosing - ticarcillin alone
CrCl above 60 mL/min: no change; 30-60: 2 g every 4 hours; 10-30: 2 g every 8 hours; below 10: 1-2 g every 12 hours; below 10 with hepatic dysfunction: 2 g every 24 hours.
Renal Dosing - ticarcillin/clavulanate
CrCl above 60: no change; 30-60: source alternatives 2 g every 4 hours or 3.1 g every 8 hours; 10-30: 2 g every 8 hours or 3.1 g every 12 hours; below 10: 2 g every 12 hours; below 10 with hepatic dysfunction: 2 g every 24 hours.
