Renal dosing, restrictions and evidence limits

The source states renal insufficiency alone does not require dose adjustment in adults and children older than three months, including ESRD. It separately warns that neonates excrete much more unchanged theophylline and require dose reduction and frequent serum-concentration monitoring when renal function declines. That neonatal pharmacology statement does not establish that this extended-release tablet is an appropriate neonatal formulation.
