Renal dosing, restrictions and evidence limits
The selected pharmacokinetic section states that renal impairment does not significantly affect terazosin elimination. Hemodialysis removes approximately 10%, and the source says supplemental dosing is unnecessary.
These statements do not remove first-dose hypotension or syncope risk. Begin with 1 mg at bedtime and titrate cautiously according to the indication and blood pressure. If treatment has been interrupted for several days, restart using the initial dosing regimen. This is not a dialysis-session blood-pressure protocol.
