XPHOZAH is a phosphate-control product for selected adults with CKD on dialysis, not a universal renal-adjustment instruction for tenapanor. Its source also contains specific separation and enalapril interaction instructions.

Before applying: Confirm the phosphate-binder context, dialysis timing and interacting medicines. The enalapril statement must not be generalized to other ACE inhibitors or used as an automatic dose increase.

Renal dosing, restrictions and evidence limits

For adult CKD patients on dialysis with inadequate response to phosphate binders or intolerance of any binder dose, the labeled regimen is 30 mg orally twice daily before morning and evening meals. Monitor phosphorus and adjust for gastrointestinal tolerability. Do not take immediately before hemodialysis; take before the next meal after dialysis because diarrhea can follow administration. Stop treatment for severe diarrhea. Separate sodium polystyrene sulfonate by at least 3 hours. Review OATP2B1 interactions, particularly enalapril: the label contains an exception to the usual lower starting-dose advice when coadministered. The dialysis-specific indication is not a general CKD or IBSRELA dosing recommendation.
