Renal dosing, restrictions and evidence limits

CrCl <30 or dialysis: 600 mg once daily; on hemodialysis days give after the session. If severe renal impairment coexists with hepatic impairment, reduce to 400 mg once daily. CrCl >=30 does not require renal adjustment in this source. Do not interpret these historical renal instructions as confirmation that telithromycin is an appropriate current antibiotic choice.

Usual Dosing (Adults)

KETEK 800 mg (two 400 mg tablets) orally every 24 hours for 7-10 days, with or without food. Hepatic impairment alone does not require adjustment in the source.

Renal Dosing

CrCl above 30 mL/min: no change. Below 30, including dialysis: 600 mg daily. Severe renal impairment plus hepatic impairment: 400 mg daily.
