Dosing / renal protocol and limits

Adult acute bacterial skin/skin-structure infection: 200 mg orally or IV once daily for 6 days. Give IV doses over 1 hour. IV-to-oral conversion does not change the dose. No renal adjustment is required, including during hemodialysis; dialysis does not meaningfully remove tedizolid.

Longer treatment is not the labeled course. Consider hematologic monitoring for prolonged off-label therapy. This adult card does not reproduce the label's separate pediatric weight-based schedules.

Historical renal / replacement-therapy guidance

The 20-50, 10-20 and <10 mL/min bands retain the usual indication-specific dose. The listed regimen is 200 mg once daily for 6 days. PD removal is described as absent and HD removal below 10%; usual dosing is retained. High-flux/continuous removal is unknown, although the book retains usual dosing. These are historical RRT statements, not contemporary measured clearance for every circuit.
