Adults at least 18 years old; HABP/VABP caused by susceptible Acinetobacter baumannii-calcoaceticus complex.

CrCl by Cockcroft-Gault, mL/min.

Renal dosing, restrictions and evidence limits

Each dose contains sulbactam 1 g plus durlobactam 1 g. Infuse every dose intravenously over 3 hours.

Adult IV HABP/VABP due to susceptible Acinetobacter baumannii-calcoaceticus complex: each dose contains sulbactam 1 g plus durlobactam 1 g, infused over 3 hours. Cockcroft-Gault CrCl >=130 mL/min: every 4 hours; 45-129: every 6 hours; 30-44: every 8 hours; 15-29: every 12 hours. Below 15 when initiating treatment: every 12 hours for the first three doses (0, 12, 24 hours), then every 24 hours. If already receiving treatment and CrCl falls below 15, use every 24 hours. Give dialysis-day doses after intermittent hemodialysis. Reassess changing renal function. This source places exactly 130 in the four-hour band. It does not supply a CRRT regimen.

Infusion preparation/duration and the rest of the combination-treatment protocol require the complete prescribing and institutional guidance.
