Renal dosing, restrictions and evidence limits
No adjustment is required with mild/moderate impairment, CrCl 30-80 mL/min. Use is not recommended below 30, including dialysis, because safety information is insufficient and exposure is prolonged.
After reversal with up to 4 mg/kg, patients with mild/moderate impairment should wait 24 hours before re-use of rocuronium 0.6 mg/kg or vecuronium 0.1 mg/kg. The source specifies rocuronium 1.2 mg/kg when an earlier block is necessary. These re-blocking instructions require anesthesia oversight; they are not additional BRIDION doses. Follow ventilation and neuromuscular monitoring requirements.
