Renal dosing, restrictions and evidence limits
Renal toxicity can be cumulative, dose-related and severe. Obtain urinalysis, BUN, creatinine, electrolytes and creatinine clearance before treatment, at least weekly during treatment, and for four weeks afterward. Quantify detected proteinuria with a 24-hour collection. Significant renal toxicity calls for dose reduction or discontinuation, but this excerpt supplies no numeric reduction algorithm.
