Renal dosing, restrictions and evidence limits

At eGFR 30-59 mL/min/1.73 m2, begin 37.5 mg once daily; maximum 75 mg once daily after at least 7 days if appropriate. At eGFR 15-29, use 37.5 mg once daily as both starting and maximum dose. Below 15, use is not recommended. Moderate/severe impairment increases the risk of blood-pressure, heart-rate and psychiatric adverse effects because half-life is prolonged. Control hypertension before treatment and monitor blood pressure regularly; monitor psychiatric symptoms. Do not convert a longer half-life into an unlisted dialysis or alternate-day regimen.

For adult use, eGFR 30-59 mL/min/1.73 m2: start 37.5 mg once daily; based on response and tolerability, increase only after at least 7 days to a maximum of 75 mg once daily. At eGFR 15-29, both the starting and maximum dose are 37.5 mg once daily. Use is not recommended below 15 (ESRD). The renal titration interval is not the general three-day titration interval. These limits do not establish a dialysis supplementation regimen.
