Renal dosing, restrictions and evidence limits

This older fixed-combination source uses Cockcroft-Gault CrCl, not the indexed eGFR bands printed in newer single-agent JANUVIA material. At CrCl 30 to below 50 mL/min, it lists a starting combination of sitagliptin 50 mg/simvastatin 40 mg once each evening; patients already using simvastatin may retain the appropriate existing simvastatin strength with sitagliptin 50 mg. At CrCl at least 50, it specifies no sitagliptin renal reduction. Below 30 or with ESRD, the product is not recommended because appropriate combination strengths are unavailable.
