Renal dosing, restrictions and evidence limits

This fixed immediate-release combination is contraindicated below eGFR 30 mL/min/1.73 m2 and is not recommended at 30 to below 45 because its sitagliptin amount cannot provide the required lower regimen. Check renal function before starting and periodically, at least annually and more often when renal risk is higher. Around iodinated contrast, follow the separate hold/reassessment instructions; renal eligibility is not determined from the metformin component alone.
