Starting doses and titration

For adult MDD or OCD, start 50 mg daily. For panic disorder, PTSD or social anxiety, start 25 mg daily. Increase by 25-50 mg no more often than weekly, according to response and tolerance, up to 200 mg/day.

Renal statement

Mild through severe renal impairment does not require a dose change. Renal impairment does not appear to materially alter sertraline exposure.

Safety context

Monitor for clinical deterioration and suicidal thoughts or behavior, particularly in pediatric and young-adult populations. Renal dose neutrality does not eliminate other contraindications or interaction risks.

Renal dosing, restrictions and evidence limits

Mild through severe renal impairment does not require a sertraline dose adjustment according to the source. The selected pharmacokinetic data support similar exposure across the studied renal groups, including hemodialysis. Study doses are not starting-dose recommendations. Hepatic restrictions, interactions and indication-specific titration remain separate.

Renal-function and dialysis summary

The book retains normal dosing in its 20-50, 10-20 and <10 mL/min bands. It also retains normal dosing for APD/CAPD, HD, high-flux HDF and CAV/VVHD. It describes removal as unlikely with peritoneal dialysis, absent with HD, and unknown for high-flux HDF and CAV/VVHD.

Source-specific limits

The normal dose must still be selected for the specific indication. The old CAV/VVHD categories are not an effluent-rate-specific modern CRRT protocol.
