Renal starting-dose modification

At eGFR below 30, start at half the usual recommended adult starting dose. For the adult tablet regimen this is sacubitril 24 mg/valsartan 26 mg twice daily. Mild or moderate renal impairment does not require a starting adjustment. Titrate according to the full adult instructions and tolerability.

Safety boundaries

Renal function, potassium and blood pressure require attention. Do not use concurrently with an ACE inhibitor; observe the labeled separation when switching. This is not a pediatric weight-based dose.

Renal dosing, restrictions and evidence limits

At eGFR <30 mL/min/1.73 m2, start at half the usual recommended starting dose, then titrate as directed by the applicable protocol. Mild or moderate impairment needs no starting-dose adjustment. This is a starting-dose rule, not a permanent half-maintenance-dose ceiling. Monitor creatinine and potassium; clinically significant renal decline may require dose reduction or interruption.
