Renal dosing, restrictions and evidence limits

For CrCl 15-59 mL/min, starting doses depend on indication. Myelofibrosis: platelets above 150 x 10^9/L, no renal change; 100-150, 10 mg twice daily; 50 to below 100, 5 mg daily; below 50, avoid. Polycythemia vera: 5 mg twice daily. Acute GVHD: 5 mg daily. Chronic GVHD: 5 mg twice daily. For ESRD on dialysis, give only after dialysis: myelofibrosis with platelets 100-200, 15 mg once; above 200, 20 mg once; polycythemia vera, 10 mg once; acute GVHD, 5 mg once; chronic GVHD, 10 mg once. The table does not supply a myelofibrosis dialysis starting dose below platelets 100. Avoid when CrCl is below 15 without dialysis. These are starting schedules, not subsequent cytopenia, interaction or response adjustments.
