Usual Dosing (Adults)
Adult influenza A prophylaxis or treatment: 100 mg twice daily. For treatment, begin promptly, preferably within 48 hours of symptoms, and continue approximately 7 days from symptom onset. Prophylaxis in children under 10 years: 5 mg/kg once daily, maximum 150 mg/day; age 10 or older: adult dose. Severe hepatic dysfunction, renal failure at CrCl at or below 10 mL/min, or elderly nursing-home patients: 100 mg daily.
Renal Dosing
The short table lists above CrCl 10: 100 mg twice daily; below 10: 100 mg daily. The longer narrative includes exactly 10 in the reduced-dose group. Multiple-dose safety data in hepatic/renal impairment are unavailable in the source. Monitor adverse effects with any renal impairment because metabolites may accumulate; adjust as needed.
Hemodialysis
100 mg every 24 hours.
