Renal dosing, restrictions and evidence limits

Renal impairment does not require a dose adjustment, but severe impairment warrants monitoring for systemic antimuscarinic effects. The source reports increased parent-drug and active-metabolite exposure at CrCl below 30 mL/min. End-stage renal disease was not evaluated. The no-adjustment statement must not be read as proof of dialysis-specific safety.
