Renal dosing, restrictions and evidence limits
The source states that the fixed-combination regimen need not be altered for renal function when its printed CrCl is above 30 mL/min/1.73 m2. It does not recommend ACCURETIC in more severe impairment, stating a preference for loop diuretics. Preserve the source's indexed unit and do not invent its handling of an exactly-at-threshold estimate. Monitor renal function, blood pressure and electrolytes; the thiazide/ACE-inhibitor combination has additional volume-depletion, hyperkalemia and interaction risks. This product-specific restriction is not proof that every thiazide-like agent lacks effect in advanced CKD.
