Usual Dosing (Adults)
Historical hydrochloride salt: 65 mg every 3-4 hours as needed for pain, maximum 390 mg/day. Napsylate salt: 100 mg every 4 hours as needed, maximum 600 mg/day.
Renal Dosing
CrCl above 10 mL/min: no change in the original text. Below 10: avoid because concentrations may rise to toxic levels.
Hemodialysis
Avoid.
