Usual Dosing (Adults)

Seizure disorders: start 125-250 mg orally at bedtime; increase by 125-250 mg/day every 3-7 days. Usual 750-1,500 mg/day in 3-4 doses; maximum 2 g/day.

Renal Dosing

CrCl 50-80 mL/min: every 8 hours. 10-50: every 8-12 hours. Below 10: every 12-24 hours.

Hemodialysis

Dose as below CrCl 10; on dialysis days add one-third of the usual dose after dialysis.
