Renal dosing, restrictions and evidence limits

For MDRD eGFR 15-29 mL/min/1.73 m2, reduce the dose to 15 mg/m2. This is a per-administration dose modification, not permission to omit the underlying cycle schedule. Avoid use below eGFR 15, with or without dialysis. The source permits an exceptional benefit-risk decision with renal monitoring and toxicity-based adjustment, but supplies no validated ESRD dose. Renal dosing, restrictions and evidence limits

The usual schedule is 30 mg/m2 IV over 3-5 minutes weekly for six weeks of a seven-week cycle. At eGFR 15-29, reduce the dose to 15 mg/m2. At eGFR <15, with or without dialysis, avoid use; exceptional benefit-risk use requires renal monitoring and toxicity-driven dose reduction. Assess CBC and mucositis at baseline and weekly; check renal/hepatic chemistry before the first and fourth doses of each cycle. These are selected renal instructions, not the complete toxicity hold/restart tables.
