The UROCIT-K renal cutoff is weight-normalized. Applying the same numeral to an automatically reported indexed eGFR would change its meaning.

Renal dosing, restrictions and evidence limits

UROCIT-K is contraindicated when GFR is below 0.7 mL/kg/min because of hyperkalemia and soft-tissue calcification risk. That weight-normalized threshold is not an indexed eGFR of 0.7 or a universal 30/60 cutoff. Renal disease also requires more frequent monitoring than the routine four-month checks of electrolytes, creatinine and blood counts. Stop for hyperkalemia, a significant creatinine increase or significant hematocrit/hemoglobin reduction. Treatment is titrated to urinary citrate and pH; a renal contraindication must not be bypassed by simply reducing the citrate dose. Pediatric safety and effectiveness are not established in this source.
