Renal dosing, restrictions and evidence limits

No renal dosage adjustment is needed. Adult moderate (Cockcroft-Gault CrCl 30-59 mL/min) and severe (<30) groups had no significant exposure changes compared with normal renal function. Dialysis effects have not been studied. Retain the complete initiation/titration and cardiac/infection precautions; this renal statement does not establish a dialysis supplement or remove nonrenal eligibility requirements.
