Renal dosing, restrictions and evidence limits

For severe renal impairment requiring hemodialysis, the reduced oral daily dose is 3 mg for multiple myeloma and 4 mg for Kaposi sarcoma. Retain the indication-specific cycle: dosing on days 1-21 of each 28-day cycle, not every day throughout the cycle. Give the dose after hemodialysis on dialysis days. The renal PK findings for nondialysis CrCl 15 to <60 mL/min do not justify transferring the dialysis reduction to every patient in those bands. These are renal starting modifications, not a substitute for hematologic, hepatic, interaction, thrombosis-prevention and pregnancy safeguards.
