Creatinine-clearance weight and dose weight are not the same

At the label's obesity threshold, Cockcroft-Gault uses ideal weight, while the mg/kg dose uses adjusted weight. Using one weight field for both would change one of the instructions.

Before applying: Document both weights and the initial versus concentration-adjusted interval. Use the trough procedure and daily renal reassessment; no dialysis or below-15 regimen is inferred.

Renal dosing, restrictions and evidence limits

Adult cUTI, including pyelonephritis: initial IV doses over 30 minutes are 15 mg/kg every 24 hours at CrCl at least 60; 10 mg/kg every 24 hours at 30 to below 60; and 10 mg/kg every 48 hours at 15 to below 30 mL/min. The label uses Cockcroft-Gault with total body weight, switching to ideal body weight when total weight is at least 25% above ideal. Dose weight differs: use total weight, or adjusted weight = ideal + 0.4 x (total - ideal) at that same threshold. Check CrCl daily. For CrCl 15 to below 90, obtain a trough about 30 minutes before dose two; a trough at least 3 mcg/mL triggers a 1.5-fold interval extension (24 to 36 hours or 48 to 72 hours). Below CrCl 15 or during renal replacement therapy, evidence is insufficient for a regimen.
