Severe renal impairment - dose reduction

For eGFR 15-29 mL/min, reduce a current 200 mg once-daily dose to 100 mg once daily. For other current doses, reduce by 50 mg; discontinue if the current dose is already 50 mg once daily. At eGFR 30-89, no renal dose adjustment is recommended.

Limit

Concomitant CYP3A inhibitors and inducers have separate modification rules. This renal table does not authorize combining adjustments by simple arithmetic or extrapolating to dialysis.
