Renal dosing, restrictions and evidence limits
The mirror specifies no renal dose adjustment, but impaired elimination can slow removal of the radioactive chelate. In heavily contaminated patients with renal impairment it describes high-efficiency, high-flux dialysis to enhance elimination; the effluent is radioactive and requires radiation precautions.
Follow contaminant-specific monitoring, blood counts, renal chemistry, electrolytes and zinc. Prolonged Ca-DTPA use has mineral-depletion concerns; the source prefers Zn-DTPA after the initial dose when further chelation is needed and it is available. This is not an interchangeable regimen for edetate calcium disodium, nor a routine dialysis supplemental dose.
