Usual Dosing (Adults)
Neuromuscular blockade: 0.06-0.1 mg/kg initially, or 0.05 mg/kg after succinylcholine for intubation; maintenance 0.01 mg/kg at 60-100 minutes, then 0.01 mg/kg every 25-60 minutes. ICU blockade: 0.05-0.1 mg/kg bolus, then 0.8-1.7 micrograms/kg/min after initial recovery, or 0.1-0.2 mg/kg every 1-3 hours.
Renal Dosing
Renal impairment doubles elimination half-life, reduces clearance and can markedly slow recovery. CrCl 10-50 mL/min: 50% of normal dose. Below 10: do not use.
