Usual dosing

Influenza treatment: adults/adolescents at least 13 years, 75 mg twice daily for 5 days. Ages 1-12 years: weight-based twice-daily dosing for 5 days; the weight table is not reproduced on this source page. Age 2 weeks to under 1 year: 3 mg/kg twice daily for 5 days.

Prophylaxis: adults/adolescents, 75 mg daily for at least 10 days, or up to 6 weeks during a community outbreak. Ages 1-12: weight-based once daily for 10 days, or up to 6 weeks in an outbreak.

CrCl above 60-90 mL/min: 75 mg twice daily for 5 days.

Above 30-60: 30 mg twice daily for 5 days.

Above 10-30: 30 mg once daily for 5 days.

ESRD on HD, CrCl at or below 10: 30 mg after each HD cycle; treatment period no longer than 5 days. This assumes three sessions in 5 days. Treatment may begin immediately during the 48 hours between sessions; give the post-HD dose regardless of when the first dose was administered.

ESRD on CAPD: one 30 mg dose immediately after an exchange. Evidence is from CAPD, not every peritoneal-dialysis modality.
