1 additional source qualifications - review before use
The label cannot recommend a dose below CrCl 50. eviQ describes PK-based no adjustment in mild/moderate impairment but gives no numerical renal bands; scope alignment remains unresolved.
Before applying: Obtain oncology/pharmacy adjudication without assigning eviQ an invented cutoff; retain the neurotoxicity stopping rule.

Renal measure
Creatinine clearance (CrCl), mL/min; equation not established in the selected dosing passage.
RX mirror: this is not independent corroboration of its underlying label. Check dated-label and formulation limitations.
Renal dosing, restrictions and evidence limits
The label recommends no dose adjustment at CrCl >=50 mL/min and states insufficient data to recommend a dose below 50. Closely monitor toxicity in moderate (30-50) and severe (<30) impairment because ara-G clearance falls. Discontinue for neurologic toxicity grade 2 or higher. The selected renal statement does not define a lower-function regimen or hemodialysis schedule; do not convert sparse pharmacokinetic observations into a calculated reduction.

Nelarabine: label evidence gap versus qualitative eviQ guidance
The label cannot recommend a dose below CrCl 50. eviQ describes PK-based no adjustment in mild/moderate impairment but gives no numerical renal bands; scope alignment remains unresolved.
How to use this: Obtain oncology/pharmacy adjudication without assigning eviQ an invented cutoff; retain the neurotoxicity stopping rule.

Renal dosing, restrictions and evidence limits
The eviQ nelarabine protocol says dedicated kidney-dysfunction studies are lacking, but pharmacokinetic analyses support no adjustment for mild to moderate dysfunction. It does not define numeric renal bands in that paragraph. Grade 2 or greater neurotoxicity requires discontinuation. Recommendations are guidance based on limited evidence and require regimen, treatment-intent and patient-specific oncology assessment. This broader qualitative statement does not supply a verified dose below the label's CrCl 50 boundary or a dialysis protocol.
