Renal dosing, restrictions and evidence limits
Severe renal impairment with CrCl <15 mL/min is contraindicated in the dosing section. In mild or moderate impairment, use a 1-mg starting dose and a maximum of 2.5 mg in 24 hours. The general 5-mg daily maximum must not be applied to this renal pathway. The pharmacokinetic discussion separately describes unstudied severe impairment as CrCl <=15; the endpoint wording differs. Do not silently reconcile the exact-15 boundary or infer a dialysis regimen.
