Usual Dosing (Adults)
Oral, opioid-naive: 50 mg every 3-4 hours as needed; usual range 50-150 mg every 2-4 hours as needed. IM/subcutaneous, opioid-naive: 50-75 mg every 3-4 hours as needed.
Not for chronic pain. For acute pain in patients without renal or CNS disease, the source limits use to 48 hours and 600 mg in 24 hours.
Renal Dosing
CrCl above 50 mL/min: standard dosing, maximum 600 mg/day for up to 48 hours.
CrCl 10-50: avoid; repeated use is discouraged because the neuroexcitatory metabolite normeperidine accumulates. The source also lists a historical cautious alternative of 75% of the usual dose for 24-48 hours, maximum 400-450 mg/day.
CrCl below 10: avoid, particularly repeated dosing because of accumulation. The source separately lists a historical alternative of 50% of the usual dose for up to 24 hours, maximum 200-300 mg/day.
