Renal dosing, restrictions and evidence limits
KALETRA renal-impairment pharmacokinetics have not been studied; negligible renal clearance suggests no important reduction in total clearance. This is not a tested dialysis regimen. Separately, with coadministered clarithromycin, the source reduces the clarithromycin dose by 50% at CrCl 30-60 and by 75% below 30. Those percentages modify clarithromycin, not the KALETRA tablet dose.
