Usual Dosing (Adults)
Heparin-induced thrombocytopenia: 0.4 mg/kg IV push over 15 to 20 seconds, then 0.15 mg/kg/hour continuous infusion. Reduce both load/infusion in renal insufficiency. Do not exceed 0.21 mg/kg/hour. Maximum initial bolus 44 mg and initial infusion 16.5 mg/hour.
Renal Dosing
CrCl >60 mL/min: no changes. CrCl 45 to 60 or serum creatinine 1.6 to 2 mg/dL: 0.2 mg/kg IV over 15 to 20 seconds, then 50% usual infusion, 0.075 mg/kg/hour. CrCl 30 to 44 or creatinine 2.1 to 3: same 0.2 mg/kg load, then 30% infusion, 0.045 mg/kg/hour. CrCl 15 to 29 or creatinine 3.1 to 6: same 0.2 mg/kg load, then 15% infusion, 0.0225 mg/kg/hour. CrCl <15 or creatinine >6: avoid or STOP infusion.
