Renal dosing, restrictions and evidence limits

For acromegaly with CrCl <60 mL/min, start 60 mg by deep subcutaneous injection every four weeks for three months, then adjust. Use caution before extending a 120-mg dose to six- or eight-week intervals in moderate/severe impairment. In GEP-NET, mild/moderate impairment did not affect clearance during 120-mg treatment, but severe impairment was not studied. The acromegaly starting reduction is not an established GEP-NET renal regimen.
