Renal dosing, restrictions and evidence limits
For renally impaired adults eligible for oral continuation, the source gives 10 mg once, then 10 mg every 4-6 hours as needed, maximum 40 mg/day. Combined parenteral plus oral duration must not exceed five days. Advanced renal impairment or risk of renal failure from volume depletion contraindicates use. No numeric clearance cutoff is supplied for that contraindication. A reduced oral dose does not override it.
