Renal dosing, restrictions and evidence limits
The 2001 source prefers serum assays in renal impairment. When assays are unavailable, it suggests 7.5 mg/kg at an interval in hours equal to serum creatinine (mg/dL) multiplied by 9. This is a historical fallback, not an automated modern aminoglycoside calculator. Reassess kidney function frequently and monitor drug concentrations and auditory/vestibular toxicity. Renal deterioration or toxicity requires dose adjustment or discontinuation. Do not apply the formula to unstable kidney function or dialysis without specialist reconciliation.
