Renal dosing, restrictions and evidence limits

Sucrose-free formulation does not eliminate acute renal failure risk, including injury associated with hemolysis. Correct volume depletion and assess kidney function before the initial infusion and periodically. For susceptible patients, use the lowest practicable dose and infusion rate and monitor urine output. The administration section instructs discontinuation if kidney function deteriorates; the precautions section says to consider discontinuation. Reassess promptly rather than treating the source as a renal safety guarantee.
