Renal dosing, restrictions and evidence limits
HIZENTRA is sucrose-free, but renal dysfunction remains a precaution. Ensure the patient is not volume depleted. In patients at renal risk, consider lower, more frequent dosing and monitor renal function and urine output. Obtain BUN and creatinine before the first infusion and at appropriate intervals; consider discontinuation if renal function deteriorates.
Risk factors include pre-existing renal insufficiency, diabetes, age over 65, volume depletion, sepsis, paraproteinemia and nephrotoxic drugs. No fixed CrCl dose-reduction table is supplied. HIZENTRA is for subcutaneous use; an IVIG rate or a different immune-globulin product schedule is not interchangeable.
