Renal dosing, restrictions and evidence limits
The absence of sucrose does not remove the risk of renal dysfunction. Avoid volume depletion, assess BUN/creatinine before the first infusion and periodically, and monitor urine output. In patients at renal risk, use the lowest practical IV rate, less than 8 mg IgG/kg/min (0.08 mL/kg/min) in this source. Consider stopping therapy if renal function deteriorates. Distinguish true hyponatremia from immunoglobulin-associated pseudohyponatremia before fluid restriction, because unnecessary restriction can worsen volume depletion and viscosity risk.
