Renal dosing, restrictions and evidence limits
Ensure adequate hydration without volume depletion before infusion. In patients at risk of renal dysfunction, use the lowest practicable IV rate and do not exceed 3.3 mg IgG/kg/min (the source also prints less than 2 mL/kg/hour). Assess BUN and serum creatinine before treatment and periodically, and monitor urine output. Risk factors include existing renal impairment, diabetes, older age, sepsis and nephrotoxic drugs. Consider stopping treatment if kidney function deteriorates. This is a product-specific infusion precaution, not a fixed reduction in the total immunoglobulin dose.
