All adult doses are total combination amounts, given IV every 6 hours over 30 minutes.

The label course is 4-14 days according to indication/response. Pediatric weight/eGFR schedules are separate.

Dosing / renal protocol and limits

Total combined IV doses every 6 hours: CrCl >90, 1.25 g; 60-90, 1 g; 30-60, 750 mg; 15-30, 500 mg. A 1.25 g dose comprises imipenem 500 mg, cilastatin 500 mg and relebactam 250 mg.

IHD: 500 mg every 6 hours. Continuous-HD dosing is not supplied. The website's adjacent renal ranges overlap at their exact endpoints; use the separately listed label boundaries when an exact 30, 60 or 90 mL/min value must be assigned.
