Birth-weight neonatal course with a urine-output hold

The dose sequence is specific to premature infants with PDA and uses birth weight. Oliguria at a later scheduled dose leads to withholding, not an invented percentage reduction.

Before applying: Retain gestational age, birth-weight eligibility, urine output, laboratory recovery and PDA reassessment. This is not adult ibuprofen analgesia guidance.

Renal dosing, restrictions and evidence limits

The labeled course is 10 mg/kg IV followed by 5 mg/kg at 24 and 48 hours, using birth weight. If anuria or urine output below 0.6 mL/kg/hour is present when dose two or three is due, give no further dose until laboratory studies show renal recovery to normal. This is a withholding rule, not a reduced adult-NSAID regimen. Review the full neonatal contraindications and PDA assessment before treatment.
