Initial renal dosing

This is a 50% initial-dose reduction. On hemodialysis days, give the dose after dialysis.

Monitoring and scope

Monitor hematologic parameters closely and adjust treatment for marrow suppression. The renal table does not replace indication-specific duration, weight selection or subsequent toxicity adjustments in the full label.
