Renal dosing, restrictions and evidence limits
In renal impairment, initiate hydromorphone injection at one-quarter to one-half of the usual starting dose, depending on the degree of impairment. Titrate cautiously and monitor closely for sedation and respiratory depression. The excerpt does not assign an automatic quarter-dose or half-dose to a specific CrCl band or supply a dialysis schedule.
The high-potency presentation has additional opioid-tolerance restrictions. Confirm the exact concentration and the complete product instructions; these injection instructions are not an oral extended-release conversion rule.
