VORAXAZE: rescue eligibility, leucovorin timing and assay artifact

Rescue is tied to delayed methotrexate elimination with kidney dysfunction. After glucarpidase, an immunoassay may remain misleading, and leucovorin timing matters.

Before applying: Follow the complete rescue pathway and validated concentration method; do not infer a repeat dose from an interfering assay.

Also check: VORAXAZE: rescue eligibility, leucovorin timing and assay artifact

Renal dosing, restrictions and evidence limits

For the specified methotrexate rescue indication, the source recommends a single 50 Units/kg IV dose over five minutes, without renal dose adjustment. Continue the accompanying hydration, urinary alkalinization and leucovorin rescue plan; separate leucovorin by at least two hours before or after glucarpidase. Methotrexate immunoassays may overestimate concentrations for 48 hours because of DAMPA interference; a chromatographic method is needed for reliable values in that period. Do not apply this rescue intervention to expected methotrexate clearance or use a laboratory artifact to invent a repeat dose.
