Renal dosing, restrictions and evidence limits
At CrCl <50 mL/min, start 18.2 mg once daily; after Day 7, response and tolerability may permit an increase to the renal maximum of 36.3 mg daily. Correct electrolyte abnormalities and obtain ECGs before treatment, during titration and periodically. Do not initiate if baseline QTc >450 ms or escalate if QTcF >450 ms. Swallow whole with food. This is not a complete interaction or psychiatric monitoring protocol.
